BSA Medical Forms – Important!
BSA Medical forms are good for one calendar year. We have many adult leaders and scouts whose form is expiring shortly or has now expired. Attached are medical forms needed. For those attending summer camp and/or the high adventure trip, all parts, A, B and C are required. If you are not attending a high adventure trip or summer camp then only parts A and B are required at this time.
Also, please attach a copy of your immunization form as well as front and back copies of your health insurance card. Please bring completed form to any scout meeting and give to Bala Rayapalli. You can also email Bala at rayapalli@gmail.com should you have a question as to whether your form has indeed expired.
So that we may everything properly recorded for high adventure and summer camp, we ask that forms be returned no later than May 1st.
