Medical Forms
As a reminder, we need updated medical forms for scouts and leaders. A new updated form from the BSA is attached. Parts A and B are needed for weekend trips. If your scout is attending a trip longer than 72 hours (Washington D.C.and/or Summer Camp) we need Parts A, B and C which is filled out by personal physician or minute clinic type of exam.
We ask that you attend to this as soon as possible since keeping up with medical forms is a time consuming task and one that we do not like handling at the last minute before a trip. Completed forms can be given to Bala Rayapalli or contact him at rayapalli@gmail.com for any questions. ALSO – Remember to attach the most current copy of your insurance card (front and back) and immunization record if available.
